You will need to bring:

1. The original Affidavit of Support and Consent document (and copies if applicable).
2. The original Special Power of Attorney document.
3. Original Birth Certificate and a photocopy of the minor child that is being accompanied.
4. Original and one photocopy of valid identification card (preferably passport or driver’s license, etc.) of the person accompanying the minor child.
5. Original and one photocopy of valid identification card (preferably passport or driver’s license, etc.) of the minor child's parent or legal guardian.
6. Applicable processing fee (non refundable) of $25.00 per document set, payable in cash.
